Severe optic disc edema. Massive optic disc edema is present in this patient with intracranial hypertension and systemic hypertension. The optic nerve is elevated and enlarged. Hemorrhage is present in the nerve fiber layer (flame-shaped hemorrhages) and within the retina (dot/blot hemorrhages). The swollen nerve elevates and laterally displaces the choroid and retina, creating concentric chorioretinal folds called Paton lines.
