She occasionally wakes up in the middle of the night to shower as well.  When she comes in for her well-child check she has red, chapped skin on both hands, and she acknowledges that she washes her hands “a lot.”  Her mother reports that she spends a total of 2 hours each afternoon, after she comes home from school, washing her hands because she believes her desk at school is “covered in germs.”

Which medication is approved by the United States Food and Drug Administration (FDA) to treat these symptoms for a 6-year-old patient?

  1. Fluoxetine (Prozac©)
  2. Paroxetine (Paxil©)
  3. Fluvoxamine (Luvox©)
  4. Sertraline (Zoloft©)
  5. Clomipramine (Anafranil©)

The symptoms described are those of obsessive-compulsive disorder.   There a number of medications approved by the FDA for treatment of these symptoms; of those listed above, only paroxetine is not approved for this use.  The distinguishing characteristic (in addition to side effect profile) to consider when selecting a medication is the age for which FDA approval has been given (Table 93-4).  Only sertraline has been approved to treat children as young as 6 years old.
