Case:

A 66-year-old patient with IDDM, COPD, coronary artery disease, hypertension, and gout presents with hypotension and malaise from his nursing home. He is febrile, and he has a urinalysis positive for nitrates, concerning for urinary tract infection as the source of his infection.
