During a routine home health visit, you come across your patient who has sustained an obvious injury to the right lower extremity and is lying on the floor in distress. What would be your most appropriate course of action? 
1. Call the home health agency and report the incident
2. Get the patient off the floor and take vital signs
3. Call 911 and wait outside for the paramedics to arrive
4.  Call 911 and then the home health agency to report the incident
